The conversation around perinatal mental health has expanded, and postpartum depression has a name now. And yet many mothers arrive in therapy describing something that went unrecognized for months, because what they were experiencing didn’t fit the image they had of what “struggling” was supposed to look like.
Many mothers are still the ones holding everything together through all of it. Holding everything together while struggling internally is exhausting in a way that is very hard to name, especially when, from the outside, everything looks like it’s going fine.

Perinatal mental health shifts are biological as much as they are psychological. Your therapist will help you understand what’s happening in your body – why the hypervigilance won’t turn off, why small things feel like threats, why you might feel disconnected even when you’re doing everything right. Processing what didn't go as planned
Birth trauma, pregnancy complications, NICU stays, pregnancy loss – these experiences often go unprocessed because there was a baby to care for immediately after. EMDR therapy creates space to go back to what happened and work through it at a pace that doesn’t overwhelm your system.

Frequently Asked Questions About Perinatal Mental Health Therapy
Do I need a diagnosis to start perinatal therapy?
No. You don’t need a formal diagnosis of postpartum depression or any other condition to begin. Many people who come for perinatal support are not in crisis – they’re somewhere in between, and they know something has shifted. If you’ve been feeling off since pregnancy or after giving birth, that’s enough of a reason to reach out.
What is the difference between postpartum depression and postpartum anxiety?
Postpartum depression typically involves a persistent low mood, emotional flatness, loss of motivation, and difficulty feeling connected to your baby or yourself.
Postpartum anxiety often presents as constant worry, difficulty sleeping even when the baby is sleeping, hypervigilance, and physical tension that runs in the background all day.
Both are common, and many mothers experience elements of both at once. Your therapist will work with what’s actually happening for you rather than fitting you into a category.
Can perinatal therapy help with intrusive thoughts about my baby?
Yes, and this experience is far more common than most mothers are told. Intrusive thoughts (fears about harm coming to the baby, images that feel completely at odds with who you are, thoughts you’d never act on but can’t stop having) are a recognized feature of perinatal OCD and postpartum anxiety.
They cause significant distress, and often significant shame, because they feel unspeakable. A therapist who understands perinatal mental health can help you name what’s happening, understand what’s driving it, and reduce its hold.
What types of therapy are used for perinatal mental health?
LIV therapists draw on CBT for postpartum anxiety and depression, EMDR for birth trauma and perinatal PTSD, and somatic approaches where there is significant body-based dysregulation. The approach is chosen based on what is actually keeping things stuck for that person, not a standard protocol applied to everyone.
How long does perinatal therapy typically take?
It depends on what’s driving things. Processing a specific experience like birth trauma or pregnancy loss often takes 8 to 12 sessions. When becoming a parent has activated longer-standing patterns, the work tends to run longer. Your therapist will give you a realistic picture after the first few sessions.
